SFDC Application Checklist

Make sure all required documents are complete and accurate before submitting. Missing or incorrect documents may delay or deny your request.

Step 1:

Application Confirmation
Sign and date the application after receiving it by email
Step 2:

Proof of Dependency
Provide either a Birth Certificate
or a Benefits Confirmation
(log in → View Full Benefits Dashboard (bottom left) → Confirmation (PDF) (top right)).
Step 3:

Grade Report
Must include the dependent’s name, school name, and completed courses with grades.
Step 4:

Billing Statement (Account Summary)
Must include the dependent’s name, school name, and a zero balance; screenshots or photos are not accepted.
