FMLA and Disability Leave: Do It Right

Do it right, avoid delays or issues. Here's what to know about FMLA and disability leave, documentation, and time off reporting.

What Happens

  • Documentation from your healthcare provider is requested with a due date

  • Leave and attendance are tracked

  • You will receive updates regarding your request

How Decisions Are Made

  • Sedgwick reviews your certification

  • Information may be verified with your healthcare provider

  • Leave usage is compared to the approved certification

FMLA and Disability Leave Documentation

Do this:

  • Treat with your licensed healthcare provider

  • Your healthcare provider completes your forms

  • Submit forms exactly as completed

Don't do this:

  • Complete the forms yourself

  • Change or add information

  • Purchase documents from a service, website, friend, or anyone where you are not being treated by the provider

Time Off and Leave Reporting

Absences, tardies, and home earlies: report it right.

Company Reporting: Call (800) 810 2271, or online at fca.fyi/callin.com

Sedgwick Reporting: Call (888) 332 4462, or online at mySedgwick.com/FCA

  • Absence: Report to the company at least 1 hour before shift start. Report FMLA or disability absence to Sedgwick.

  • Tardy: Report to the company at least 30 minutes before shift start. Report FMLA tardy to Sedgwick.

  • Home Early (FMLA): Notify your supervisor before leaving work. Contact Sedgwick before leaving work, unless it is an emergency.

Important Reminders

  • Provide complete, accurate, and timely information

  • Follow all required reporting and documentation procedures

  • Submit documentation exactly as completed by your licensed healthcare provider

  • Failure to meet these requirements may violate company policy and may result in disciplinary action, denial of leave, delays in processing, or other consequences
